Client Contract

Permission for Massage and Bodywork

I, ________________________________ (printed name) hereby give permission for Cassie Brown LMT of Mindful Massage and Bodywork LLC to perform massage and bodywork on me in the ways that are discussed prior to each session and acknowledge there may be pain and soreness in the process and in the few days after the work has been performed and release Cassie Brown from liability for that pain and soreness. If there is any problem, I will discuss it with Cassie Brown in person or via text or email prior to any legal claims.

Cancellation/No Show Policy

I also acknowledge and agree to her cancellation policy as it stands hereafter.

  • Same Day Cancellation: 100% cost of massage time booked

  • Less than 24 hours notice: 50% cost of massage time booked

  • More than 24 hours notice: 0% cost of massage time booked

These fees must be paid before I will be booked for another massage or bodywork session.

Good Behavior Contract

I, ________(Initial) also agree to and acknowledge the Good Behavior Contract that stands hereafter.


I will not be inappropriate in any way verbally or physically towards Cassie Brown LMT of Mindful Massage and Bodywork, LLC. There will be no touching of Cassie Brown LMT on any part of her body.


There will also be no inappropriate language or behavior including, but not limited, to sexual comments, flirting, inappropriate noises while massage and bodywork are being performed, removing clothing before Cassie Brown exits the room for you to disrobe and lay down on the table UNDER the top sheet. There will be no exposing yourself in any way to Cassie Brown. If Cassie Brown feels uncomfortable or violated in anyway the first offense will be a verbal warning followed by a write up after the session. If behavior continues or worsens the session will be immediately ended and I will be written up. After this, there will be one more chance given, and if any behavior continues or worsens I understand I will be terminated as a client.
